Diphteria, Tetanus Toxoid, and Pertussis Vaccine:   Three doses of any combination. (DTP, DTaP, DT,Td )   One dose of Td required ten years after last dose.
Polio: (IPV) Three or four doses.  If third dose on or after 4th birthday don't need a fourth.
Measles, Mumps, Rubella: Two doses measles and one of (MMR) mumps and rubella.
Hepatitis B:   Three doses are required 
Varicella:   One dose or a written statement from parent, physician or school nurse.
